Job Title: Adult Community Care Officer

We are seeking a skilled and compassionate Adult Community Care Officer to join our team at Nottinghamshire County Council. As a key member of our Improving Lives AMHP Service, you will play a vital role in supporting individuals with enduring mental illness/crisis, promoting recovery and prevention principles, and upholding human rights.

About the Role:
  • Provide a structured strength-based, reablement-focused approach to support individuals with mental health issues.
  • Collaborate with multi-agency partners to develop pathways of least restrictive alternatives to hospital admission and strengthen community support.
  • Contribute to the development of 'wrap-around' support services, promoting a whole-system approach to bed management and community support.
  • Work closely with individuals, family/carers, health colleagues, and other key partners to ensure effective support and care.
Requirements:
  • Experience working with individuals with mental health issues and a good understanding of reablement and risk assessment/risk enablement.
  • Strong communication skills and ability to work collaboratively with diverse stakeholders.
  • Commitment to the principles of mental health legislation and the promotion of human rights.
