Join Our Team as a Community Care Specialist
We are seeking enthusiastic, skilled, and dedicated Community Care Specialists to become integral members of our Ageing Well / Living Well Community Teams.
As part of a well-established community team, you will provide support to individuals residing in the community who may be experiencing physical disabilities and/or cognitive challenges. This role presents a unique opportunity to contribute to a dynamic Social Care team focused on enhancing and evolving service delivery.
Key Responsibilities:
- Possess a recognized Social Work qualification and be registered with Social Work England.
- Demonstrate experience and understanding of the Mental Capacity Act.
- Have knowledge of Care and Support assessments, needs, and criteria as outlined in the Care Act.
- Experience in conducting Section 42 inquiries, achieving preferred outcomes, and promoting personalized safeguarding.
- Exhibit the ability to work independently as well as collaboratively within a team, engaging with partner organizations and community health teams.
- Manage a diverse caseload of vulnerable adults facing various complex conditions, including conducting Mental Capacity assessments, Decision Support Tools (DSTs), and safeguarding responsibilities.
- Understand Nottinghamshire's Adult Care Strategy and relevant policies and legislation that inform social work practices.
- Possess strong communication and organizational skills.
