Job Description

Belmont Recruitment is seeking an experienced Community Care Officer to work with a Local Authority on a temporary basis. This is a full-time role working 37 hours per week (Monday to Friday) with hybrid working arrangements following initial training.

Main Responsibilities:
  1. To conduct comprehensive assessments of adults 18 years and over with diverse needs, utilizing a holistic and person-centered approach.
  2. To ensure the delivery of appropriate services to safeguard vulnerable adults and their families, adhering to assessed needs and departmental eligibility criteria.
  3. To undertake assessments and create culturally sensitive care plans, demonstrating flexibility and innovative resource utilization.
  4. To manage a caseload, including risk management, monitoring, coordinating, and reviewing care plans and ongoing service provision.
Essential Requirements:
  1. Experience working with adults in a social care setting.
  2. Experience working with adults facing various challenges, including learning disabilities, mental health issues, substance misuse, and physical health problems.
  3. Awareness of safeguarding and ability to identify individuals at risk, with flexibility, resilience, and prioritization skills.
  4. Ability to conduct adult-focused assessments within a person-centered approach.
  5. Experience creating and implementing tailored care plans that demonstrate flexibility and innovative resource utilization.
  6. Experience with costing and commissioning care packages.
