Position Overview

An exciting opportunity has emerged within the Redbridge Reablement Service for a new initiative that spans both hospital and community settings. The successful candidate will be responsible for conducting assessments of clients in hospital environments, initiating the Reablement process to mitigate bed paralysis and deconditioning caused by inactivity. Additionally, this role will involve community assessments and working closely with clients under our care. Flexibility and adaptability are essential for this position.

Key Responsibilities

  • Provide service users and their families with relevant information and education, ensuring they have meaningful choices that enhance dignity, independence, and quality of life.
  • Support practice with evidence-based methods, literature, and peer reviews.
  • Work independently in clinical and community settings while maintaining access to support and supervision.
  • Maintain statistical records as required by the organization, including timely data entry into the RIO system.
  • Participate in team meetings and contribute to locality discussions.
  • Document and report any incidents or accidents involving service users or staff members.
  • Ensure the care and proper maintenance of issued equipment, reporting any defects or losses.
  • Identify specific needs of service users and arrange for appropriate assessments and funding.
  • Be aware of the financial implications of recommendations and adhere to eligibility criteria set by the Trust.
  • Carry out delegated tasks to support team operations as directed by the Team Leader and colleagues.
  • Contribute to the efficient use of resources by ensuring supplies are available and suggesting ways to minimize waste.

Onboarding Process

We place great importance on welcoming new team members and ensuring they are properly inducted into the Trust. New starters will join on the first Monday of each month and will undergo a comprehensive induction program lasting up to two weeks, which includes mandatory training, systems training, and equipment allocation. During this process, new employees will have the opportunity to meet with the executive team and senior managers, as well as attend various engagement and wellbeing sessions.

Probationary Period

This position is subject to a probationary period. Internal applicants may be exempt unless they are currently undergoing a probationary period or are part of the bank staff.

High Cost Area Supplement

This role includes a High Cost Area Supplement of 15% of the basic salary, with a minimum of £4,551 and a maximum of £5,735.

Detailed Job Responsibilities

  • Utilizing a reablement approach, the post holder will promote service user independence and empowerment, assisting them in learning or regaining skills related to personal, domestic, and social tasks as part of their therapeutic plan.
  • Conduct detailed telephone and reablement assessments.
  • Ensure that service users and their carers are fully informed of their rights and have access to all relevant information throughout the assessment and reassessment process.
  • Provide holistic care to service users in various settings, including their homes, health centers, and residential facilities.
  • Manage a delegated caseload appropriate to the level of expertise.
  • Plan and organize own delegated tasks effectively.
  • Brief the Team Lead on relevant developments for each service user and present cases for closure.
  • Participate in a duty rota to provide care for service users, including during unsociable hours.
  • Assist in medication administration as necessary, following appropriate training.
  • Adhere to Trust protocols and procedures, particularly concerning manual handling.
  • Maintain accurate records of information required for audit purposes.
