Job Summary:

We are seeking a highly motivated and experienced Health and Wellbeing Coach to join our team at NHS Midlands and Lancashire Commissioning Support Unit. As a Health and Wellbeing Coach, you will work with individuals and families to support them in achieving their health and wellbeing goals.

Key Responsibilities:

  • Work with GPs and the PCN team to identify and engage a caseload of clients who would benefit from proactive support to family or household wellbeing.
  • Manage and progress clients to achieve positive outcomes based on a solution-focused approach.
  • Assess and identify levels of risk, vulnerability, and safeguarding using prescribed assessment tools, and escalate to GPs as necessary.
  • Deliver targeted activity working either on a 1-1 basis or in groups using a holistic approach to address the root causes in line with the localities outcome framework and health and wellbeing coach framework.
  • Maintain accurate records of all interactions using the PCN or council systems as required.
  • Undertake home visits to engage participants and ensure that Sefton residents are safeguarded within the community.
  • Increase parental engagement to strengthen family relationships and support children's education.
  • Work co-operatively with colleagues and partners to share information, ensuring a joined-up approach.
  • Ensure Quality Assurance Framework, legislation, regulations, policies, and procedures are central to all good practice.
  • Engage in the development of the service and CPD through PDR, training team/service meetings, and supporting raising awareness sessions across the partnership on key and emerging issues.
  • Maintain quality Creche provision and work in partnership with families to support parental understanding of their role in their child's development by modelling high-quality early education practice within the creche.
  • Signpost parents to activities within the community to build resilience within the family. Support family members to develop skills to enable them to carry out household tasks such as budgeting, supporting health and hygiene, increasing personal and parental confidence and capacity.
  • Utilise appropriate marketing techniques to promote awareness of the service and represent the service at events as required, including contributing to COMMs/Social media.
  • Ensure that client information data is lawfully gathered, accurate, up-to-date, and only divulged in accordance with GDPR.

Requirements:

  • Level 3 Childcare Qualification
  • Able to identify levels of risk, vulnerability, and needs.
  • A broad knowledge of safeguarding procedures and when to implement the escalation policy.
  • Knowledge and awareness of issues which may have an impact on families such as domestic violence, abuse, mental health difficulties, substance misuse, poverty, early years development, and housing issues.
  • Able to identify what is needed to provide a nurturing, stimulating environment for children.

Desirable Criteria:

  • IT and comms skills, including competence in the use of social media platforms, Canva, Microsoft Excel, and Word, and EMIS system.
  • Previous experience working in early help teams or as a health and wellbeing coach.

Working for Our Organisation:

South Sefton PCN is a large Primary Care Network, covering the four localities in south Sefton with a population of around [Removed] residents.

PCNs are groups of GP practices working with a wide range of partners to improve the health of their population. Key to this purpose is strengthening general practice provision through working together, where it makes sense to do so, and working as part of integrated care teams which work collaboratively for and with local people in each locality.

The PCN is participating in an innovative programme with Sefton Council to integrate our offer of proactive support to residents.

Special Conditions:

  • The post holder will be expected to move between locality delivery points depending on the needs of the service.
  • It is envisaged that the future service will encompass some evening and weekend working to meet the needs of the children, young people, and their families.
  • Since confidential information is involved with the duties of this post, the post holder will be required to exercise discretion at all times and to observe relevant codes of practice and legislation in relation to data protection and personal information.
  • Undertake, and participate in training, coaching, and development activities, as appropriate.
  • The person appointed will be expected to work flexibly, and the exact nature of the duties described above is subject to periodic review and is liable to change.
