About the Role

We are seeking a highly skilled and dedicated Community Staff Nurse to join our team at Southern Health NHS Foundation Trust. As a Community Staff Nurse, you will play a vital role in providing high-quality patient care and contributing to the provision of community-based services.

Key Responsibilities
  • Manage a defined community caseload, working with patients in their homes and clinics to provide holistic assessments and care plans.
  • Work with a high degree of autonomy and professional accountability, ensuring referrals are handled according to clinical need and care meets acceptable standards of patient safety and quality outcomes.
  • Collaborate with other healthcare professionals to provide comprehensive care and support to patients, promoting health protection and promotion programs that improve and reduce inequalities.
  • Contribute to the development and implementation of care plans, ensuring they are tailored to individual patient needs and aligned with Trust and professional record-keeping standards.
