About the Role

We are seeking a highly motivated and compassionate Support Coordinator to join our team at Home Group. As a Support Coordinator, you will play a vital role in supporting our customers who have complex needs to develop their skills and live more independently in the community.

Key Responsibilities

  • Creating support plans with customers and coordinating our small team of Support Workers to work towards these plans.
  • Working with adult females who have experienced or are at risk of experiencing homelessness or rough sleeping.
  • Helping customers with daily living activities such as making meals, shopping, budgeting, and maintaining their tenancy.
  • Carrying out risk assessments, support planning, goal setting, and regular reviews.
  • Providing emotional support and building strong relationships with customers.

Requirements

  • Experience of coordinating and assessing customer referrals.
  • The ability to work on your own initiative, remain calm under pressure, and have a resilient approach.
  • Excellent communication and interpersonal skills.
