Job Title: Support Coordinator

Home Group Limited is seeking a Support Coordinator to join our team at Tyneside Foyer in Newcastle upon Tyne. As a Support Coordinator, you will play a vital role in supporting the smooth running of our service, working with young people aged 16-25 who require emotional support, independent living skills, and advocacy.

Key Responsibilities:

  • Creating support plans with customers and coordinating the small team of Support Workers working to the plans.
  • Working with customers with complex mental health needs, helping them with daily living activities such as budgeting, maintaining their tenancy, and moving onto independent accommodation if possible.
  • Carrying out risk assessments, support planning, goal setting, and regular reviews.
  • Supporting our Support Workers in service delivery and focusing on our service KPIs, working alongside management to achieve these.

Requirements:

  • Passion to support customers to live their best life, working collaboratively with an eye for detail.
  • Experience of creating person-centred support plans and supporting colleagues in working to the plans.
  • Experience of coordinating and assessing customer referrals.
  • The ability to work on your own initiative, remain calm under pressure, and have a resilient approach.
