About the Role

We are seeking a skilled Adult Social Care Worker to join our Single Point of Access team. As a key member of our team, you will be responsible for screening incoming referrals and making sound, evidence-based decisions to meet the needs of our residents.

This is a challenging and rewarding role that requires strong time management skills, good communication, and the ability to work in a fast-paced environment. You will have the opportunity to work with a diverse team of health and social care professionals and support colleagues, and will be supported by senior practitioners and access to appropriate training and development.

The Single Point of Access team is a dynamic and forward-thinking service that is committed to promoting independence and modernizing our services to meet the demands of the future. We are looking for someone who is passionate about delivering excellent outcomes for our residents and is committed to working in a strengths-based, dynamic service delivery model.

As a member of our team, you will have the opportunity to develop your skills and experience, and will be part of a team that is making a positive contribution to the expansion of the Triage Service within the SPA. Various hours are currently available, up to full time.

This post is exempt under the provisions of the Rehabilitation of Offenders Act (Exceptions) Order 1975 and is subject to an enhanced Disclosure and Barring Service check.
