About the Role

We are seeking a highly skilled and dedicated Adult Social Care Worker to join our team in North Somerset. As a key member of our Adult Social Care Locality Team, you will work closely with Deaf/Deafblind individuals, providing support and guidance to help them achieve their goals and improve their overall wellbeing.

Key Responsibilities
  • Develop and maintain strong relationships with Deaf community members, understanding their needs and preferences.
  • Conduct Care Act Assessments and create personalized care plans to support individuals in achieving their desired outcomes.
  • Collaborate with health and social care colleagues, as well as voluntary sector agencies, to ensure seamless support for Deaf community members.
  • Organize Deaf Awareness Week annually, promoting education and understanding within the community.
  • Provide support and information to Deaf community groups, addressing their needs and concerns.
  • Identify and address equipment needs for Deaf/deafblind/hard of hearing individuals in their home environment.
Requirements
  • Passion for working with the Deaf community in a strength-based way.
  • Good knowledge of Deaf awareness and British Sign Language (BSL) at level 2 or above.
  • Background or understanding of health and social care or transferable skills related to the job description.
  • Excellent IT skills to enable training on various in-house systems.
  • Valid driving license and own transport, or alternative suitable method of travel.
