The Single Point of Access team provides timely advice, information, and triage of referrals for Adult Social Care and Health Services. Our strengths-based, dynamic service delivery model requires confident individuals to make sound, evidence-based decisions.

This challenging role involves screening incoming referrals, aiming for immediate resolution or triage for further assessment. As part of an integrated health and social care team, you will develop your skills and experience, supported by Senior Practitioners and access to relevant training.

About the Role

You will have the opportunity to work in a quick-moving environment, making decisions to meet individual needs efficiently and effectively. Various hours are available, up to full-time.

This post is exempt under the Rehabilitation of Offenders Act (Exceptions) Order 1975 and subject to an enhanced Disclosure and Barring Service check.
