We are seeking a skilled and compassionate Adult Social Care Worker to join our team in North Somerset. As a key member of our Adult Social Care Locality Team, you will work closely with Deaf/Deafblind individuals, providing support and guidance to help them achieve their goals.

Key Responsibilities:
  • Develop and maintain strong relationships with Deaf community members, understanding their needs and preferences.
  • Conduct Care Act Assessments, creating personalized care plans that promote independence and well-being.
  • Collaborate with health and social care colleagues, voluntary sector agencies, and other stakeholders to ensure seamless support for Deaf individuals.
  • Organize Deaf Awareness Week events, promoting inclusivity and understanding within the community.
  • Provide information, advice, and support to Deaf community groups, addressing their unique needs and concerns.
Requirements:
  • Passion for working with Deaf individuals, promoting a strength-based approach.
  • Good knowledge of Deaf awareness, with a preference for British Sign Language at level 2 or above.
  • Background or understanding of health and social care, with transferable skills relevant to the role.
  • Excellent IT skills, with the ability to learn in-house systems.
  • Valid driving license and own transport, or alternative suitable method of travel.
