About the Role
We are seeking caring and compassionate individuals to join our team as Care Professionals. As a Care Professional, you will work in partnership with other health professionals and agencies to meet the needs of our residents. You will have access to their support plan, which will provide you with all the information you need to do the job.
Main Responsibilities
- Promote patient-focused care and assist residents with activities of daily living, promoting their comfort.
- Deliver support plans and safeguard vulnerable individuals.
- Contribute to outcome-focused reviews and encourage residents' independence, choice, health, and wellbeing.
- Encourage the use of equipment and assistive technology, and facilitate transitions to alternative services and support if needed.
- Engage in organisational learning and development, and provide excellent patient care and support.
- Record and complete documentation and feedback.
