As a Care Coordinator, you will play a vital role in supporting the delivery of our Population Health Management Programme, which aims to provide proactive and holistic care for individuals with mild to moderate frailty.

Key Responsibilities
  • Manage a caseload of patients with mild and moderate frailty, providing support and guidance to help them navigate the healthcare system.
  • Work closely with multidisciplinary teams, including GPs, district nurses, therapists, and social care professionals, to coordinate patient care and ensure seamless transitions.
  • Develop and maintain effective relationships with patients, carers, and healthcare professionals to ensure that patients receive the best possible care.
  • Conduct regular check-ins with patients to assess their needs and provide support, ensuring that their care plans are regularly reviewed and updated.
  • Identify and address health inequalities, providing feedback on areas for improvement and contributing to the development of strategies to enhance patient engagement.
  • Support the implementation of the Population Health Management Programme, working within governance structures and providing feedback on the model's effectiveness.
Requirements
  • Relevant health and social care experience, preferably in Primary Care or a related field.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, carers, and healthcare professionals.
  • Ability to work effectively in a team environment, with a focus on patient-centered care and collaboration.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Knowledge of local health and care infrastructure, with the ability to navigate and access support services.
