As a newly established division within the Integrated Care Board, we manage interconnected areas such as elderly care, residential facilities, the development of Integrated Neighborhood Teams, immediate access to urgent care, and local healthcare commissioning. Key Responsibilities
The successful candidate will play a pivotal role in supporting and executing projects and initiatives linked to our health improvement program and its associated priorities across the system, community, and neighborhoods.
Responsibilities include:
- Facilitating the redevelopment of commissioned services through outcome-focused commissioning to enhance the development of Integrated Neighborhood Teams, improve access to health and care services, and achieve better outcomes for residents.
- Collaborating with a motivated team to manage projects that contribute to successful service redesign initiatives.
- Demonstrating experience in a dynamic and challenging environment, working collaboratively across organizational boundaries with partners and community members to implement transformational designs.

Job Duties
The role involves supporting and delivering projects aimed at enhancing the integration, coordination, performance, and clinical effectiveness of services as part of our health improvement program. Key duties include:
- Assisting in the delivery of programs, projects, and initiatives associated with the health improvement program and its priorities across the system, community, and neighborhoods, with a focus on service integration and improved patient outcomes.
- Supporting various programs and projects within the directorate's portfolio, including integrated service commissioning, transformation initiatives, and workforce development.
- Collaborating with the Strategic & Integrated Commissioning Manager to achieve outcomes-based service commissioning.
- Actively managing project delivery and reporting aligned with the health improvement program.
- Providing support to emerging Integrated Neighborhood Accelerator Sites.
- Ensuring optimal resource utilization across the system, identifying and developing plans for system improvements while monitoring and evaluating project impacts.
- Developing and implementing action plans related to agreed priorities and commitments.
- Identifying community needs and gaps in service provision, including addressing health inequalities.
- Fostering strong working relationships with various teams and organizations across the Integrated Care System to enhance health and care delivery and improve patient outcomes.

- A relevant degree-level qualification or equivalent professional experience.

- Project management qualifications such as QSIR or PRINCE2.
