Key Responsibilities
  • Deliver High-Quality Occupational Therapy Assessments and Interventions

Conduct comprehensive assessments and develop individualized care plans to address the complex needs of inpatients with a range of conditions, focusing on general medicine, frailty, and discharge planning.

Develop and Implement Individualized Care Plans

Collaborate with the multidisciplinary team to develop and implement care plans that prioritize patient-centered care and ensure continuity of care.

Facilitate Safe and Effective Discharge Planning

Liaise with community services and other healthcare providers to ensure seamless transitions and continuity of care for patients.

Provide Education and Support to Patients and Families

Offer education, advice, and support to patients, families, and carers regarding occupational therapy interventions and goals.

Contribute to Policy Development and Implementation

Participate in the development and implementation of policies, procedures, and guidelines that support delivering effective and safe occupational therapy services.

Collaborate with the Multidisciplinary Team

Actively participate in team meetings, case conferences, and clinical supervision to promote best practices and continuous professional development.

Requirements
  • HCPC-Registered Occupational Therapist

Must be a registered Occupational Therapist with the Health and Care Professions Council (HCPC).

Relevant Band 6 Experience

Must have relevant experience in an acute NHS setting, particularly in general medicine, discharge planning, and managing frailty.

Excellent Communication and Organizational Skills

Must possess excellent communication, organizational, and time management skills to work effectively in a fast-paced environment.

Ability to Work Independently and as Part of a Team

Must be able to work effectively both independently and as part of a multidisciplinary team.

Commitment to Patient Care and Continuous Improvement

Must be committed to delivering high standards of patient care and a proactive approach to continuous improvement.
