About the Role

We are seeking a highly motivated and confident Community Access Practitioner to join our team. The successful candidate will work with adult males within HMP Full Sutton or Millsike, providing support and guidance to individuals with mental health difficulties.

Main Responsibilities
  • Oversee prisoner pathways from reception to release, ensuring a smooth transition and effective reintegration into the community.
  • Develop strong therapeutic relationships with service users, offering consistent engagement and support from initial referral to identified Mental Health/LD provision.
  • Forge working alliances with local inpatient and community partners to develop effective reintegration pathways for patients.
  • Deliver high-quality care to a caseload of complex mental health and learning disability offenders as part of a liaison and in-reach provision.
Requirements
  • Detailed understanding of relevant policy, legislation, and drivers, including the Mental Capacity Act, Health & Safety, and Infection Control.
  • Member of organisational group/network, locally or nationally.
  • Evidence of continuing professional development relevant to the professional lead area, to an advanced level of post-graduate diploma or equivalent knowledge/experiential learning.
  • Experience of organisational and professional networking.
  • Advanced management/leadership training/qualification or equivalent experience.
  • Proven experience of developing others through education, mentorship, coaching, teaching, assessing, presentations, and publishing.
  • Ability to manage own time and projects effectively.
  • Ability to work within a culture of improving working lives and working time directive.
  • Ability to demonstrate ethical values and attitudes within a culture of equality and diversity.
