About the Role

We are seeking a highly skilled and experienced Dementia Care Manager to join our team at Gilbert Meher Ltd. As a key member of our care services team, you will be responsible for leading and managing our dementia community, ensuring that our residents receive the highest quality care and support.

Key Responsibilities

  • Lead and manage the dementia community, ensuring that all care plans are person-centred and of high quality.
  • Act as an ambassador for our dementia strategy, promoting high standards of relationship-centred dementia care at all times.
  • Work closely with nursing and care teams to ensure that individual care plans meet regulatory standards and are aligned with our company's policies and procedures.
  • Take ownership of team performance, ensuring that all team members have the necessary resources and support to deliver high-quality dementia care.
  • Liaise and build relationships with external professionals, including relatives and other stakeholders.
  • Champion relatives' involvement in all aspects of care and care planning, including life story work and family meetings.
  • Work in partnership with our regional dementia relationship practitioner to develop high-quality relationship-centred dementia care.
  • Plan and deliver regular opportunities for team members to work alongside the care team on shifts, providing support and coaching.
  • Ensure that all team members complete mandatory training within specified timeframes.
  • Manage the performance of team members, ensuring regular reviews and appraisals take place.
  • Take ownership of and lead on induction training for new team members, including specific dementia care induction.

Requirements

  • Hold a minimum level 3 qualification in dementia care or have equivalent experience and knowledge of evidence-based practice in dementia care.
  • Have a thorough working knowledge of care standards aligned to National Regulation and experience of the CQC registration & inspection process.
  • Have theoretical knowledge and practical experience of implementing psychosocial interventions for people living with dementia.
  • Have theoretical knowledge and practical experience of supporting people living with dementia who express needs through behaviour.
  • Have a working knowledge of safeguarding legislation, DoLS, and management of referrals.
  • Have evidence of CPD in relation to dementia care.
