About the Role
We are seeking a highly skilled and compassionate District Nurse/Community Nursing Sister/Charge Nurse to join our innovative and successful community nursing team in Newbury. This role offers an exciting opportunity to make a real difference in the lives of patients within their own homes.
Key Responsibilities
- Provide comprehensive nursing care to a diverse caseload of patients, encompassing wound management, bladder and bowel care, palliative and end-of-life support, and chronic disease management.
- Collaborate effectively with multidisciplinary teams, including GPs, social workers, and therapists, to ensure holistic patient care.
- Demonstrate strong clinical judgment, critical thinking skills, and the ability to adapt to changing patient needs.
- Maintain accurate and detailed patient records, adhering to strict confidentiality protocols.
- Promote patient education and empower individuals to manage their health effectively.
Essential Requirements
- Registered Nurse - Adult qualification (NMC registration).
- Proven experience in community nursing, with a strong understanding of relevant legislation and guidelines.
- Excellent communication, interpersonal, and problem-solving skills.
- A full UK driving licence and access to a vehicle for work purposes.
