Job Summary

District Community Nursing Sister/Charge Nurse - Band 6

Salary Range: £33,338 to £47,084

Location: Reading, Newbury, Wokingham, Bracknell, Slough, Ascot, Windsor & Maidenhead.

We are seeking an experienced and enthusiastic nurse to join our team as a District Community Nursing Sister/Charge Nurse. In this role, you will provide support and leadership to a great team and join a team that provides a catalogue of care and pride themselves on their commitment to the well-being of the community.

Main Duties of the Job

We are looking for a nurse who is willing to undertake relevant training, has leadership experience, and is able to demonstrate innovative practice. The successful candidate will be responsible for ensuring high-quality evidence-based care for patients, their families, and carers in the community.

Responsible for a complex and diverse caseload, actively working to prevent admission to the acute sector and supporting patients in their preferred place of care.Responsible for maintaining detailed, accurate, and confidential patient records, adhering to Trust policies.Leading an effective supportive team, working together to provide clinical advice and support to other staff and patients.Responsible for the management of a defined caseload of patients, with continuing responsibility for the assessment of care needs, the development, implementation, and evaluation of care programmers.Accountable for the standards of care and for determining the activity of your team. This will include acting as a role model for staff, sharing learning and developing practice.
