Position Overview
West Kent Primary Care (WKPC) is on the lookout for a dedicated Senior Nurse Practitioner (SNP) to join our esteemed Weald Primary Care Network (PCN). The SNP will play a pivotal role in delivering exceptional healthcare services to patients, including those residing in local care facilities.
Key Responsibilities
The successful candidate will be a compassionate and skilled Senior Nurse Practitioner (SNP) who will be integral to our supportive PCN team. The SNP will oversee and deliver high-quality healthcare to patients, particularly focusing on residents in care homes. Responsibilities will encompass:
- Conducting comprehensive assessments, utilizing advanced clinical judgment and critical thinking to formulate treatment plans.
- Prioritizing patient needs and triaging accordingly, ensuring timely referrals for necessary investigations in alignment with established protocols.
- Collaborating effectively with all stakeholders to fulfill patient requirements, while upholding organizational policies and providing nursing leadership as needed.
- Emphasizing the four pillars of advanced practice: Clinical Practice, Leadership, Education, and Research.

Job Duties
- Conduct regular care home visits under the guidance of the attached GP.
- Perform initial assessments for new patients in care homes, reviewing care directives and collaborating on treatment strategies.
- Manage chronic health conditions of care home residents in partnership with the care home GP.
- Implement strategies to reduce hospital admissions by monitoring high-risk residents.
- Work collaboratively within a multidisciplinary team to ensure safe and effective patient care.
- Utilize advanced clinical skills to deliver appropriate and safe nursing care.
- Assess, diagnose, and evaluate treatment plans for patients with complex health needs.
- Triaging patients and coordinating referrals to other healthcare professionals as necessary.
- Interpret diagnostic results to inform treatment decisions and consult with GPs when needed.
- Identify and manage patients at risk of developing long-term health conditions.
- Coordinate care delivery, ensuring involvement of patients and their families as appropriate.
- Prescribe and review medications in accordance with best practices and guidelines.
- Diagnose and manage both acute and chronic health issues, integrating lifestyle and pharmacological interventions.
- Request and interpret diagnostic tests as per established protocols.
- Provide care in various settings, including patient homes and care facilities.
- Facilitate referrals to other healthcare professionals and organizations as required.
- Deliver care in accordance with national guidelines and evidence-based practices.
- Maintain patient confidentiality and adhere to information governance policies.
- Demonstrate respect for patient privacy in all interactions.
Qualifications and Experience
Essential:
- Significant post-registration nursing experience.
- Recent experience in primary or community nursing.
- Experience in managing minor illnesses and injuries.
- Proficiency in nurse-led triage.
- Experience in developing clinical protocols and guidelines.
- Leadership experience in clinical settings.
- Involvement in clinical audits and research.
- Qualified in nurse prescribing.
Desirable:
- Experience in project management.
- Skills in health needs assessment.
Skills and Knowledge

- Strong clinical leadership abilities.
- Excellent communication skills, both written and verbal.
- Ability to convey complex information to patients and families.

Qualifications:
Essential:
- Registered first-level nurse with a relevant nursing or health degree.
- MSc in advanced practice or equivalent qualification.
- Independent prescribing certification.

- Mentorship or teaching qualifications.
- Training and experience in clinical supervision.
