Job Title: Nurse Practitioner for Homeless Population

We are seeking a skilled and compassionate Nurse Practitioner to join our team in providing high-quality healthcare services to individuals experiencing homelessness.

About the Role:
  • Conduct comprehensive physical and psycho-social assessments for individuals in our care.
  • Develop and implement individualized care plans to address the unique needs of each client.
  • Provide clinical and therapeutic treatments, including screening for blood-borne viruses and vaccinations as needed.
  • Work collaboratively with community teams and voluntary organizations to ensure seamless delivery of care.
  • Act as an advocate for individuals experiencing homelessness, promoting independence and self-care.

Requirements:
  • Registered Nurse with current NMC registration.

  • Ability to work collaboratively as part of a multidisciplinary team.
