Job Title: Assertive Outreach Nurse

We are seeking a skilled and compassionate Assertive Outreach Nurse to join our team at Change Grow Live. As an Assertive Outreach Nurse, you will play a vital role in providing high-quality care and support to individuals who are homeless or at risk of homelessness.

Main Responsibilities:
  • Conduct comprehensive assessments of service users to identify their physical and psycho-social needs.
  • Develop and implement personalized care plans to address the complex needs of service users.
  • Provide clinical interventions, including wound care and basic wound dressing, to improve health outcomes for service users.
  • Work in partnership with other community teams and voluntary organizations to ensure seamless delivery of care to service users.
  • Act as an advocate for service users, promoting their independence and self-care.
  • Contribute to the development of project initiatives to improve health outcomes for service users.
Requirements:
  • Registered Nurse with a valid nursing license.
  • Experience working with homeless populations and individuals with complex needs.
  • Strong assessment and planning skills.
  • Ability to work in a fast-paced environment and prioritize tasks effectively.
  • Excellent communication and interpersonal skills.
