Job Title: Community Mental Health Practitioner

We are seeking a skilled and compassionate Community Mental Health Practitioner to join our Homeless Health Care Team in Southampton.

Main Responsibilities:
  • Provide holistic care and support to individuals experiencing homelessness, with a focus on mental health and wellbeing.
  • Care-coordinate a small caseload of service users, assessing and formulating individual intervention plans to address complex biopsychosocial problems.
  • Work in partnership with colleagues to evaluate care plans and contribute to weekly multidisciplinary team meetings.
  • Provide supervision arrangements to support practitioners in developing their skills and knowledge.
Requirements:
  • Qualified as a registered mental health nurse, occupational therapist, or social worker.
  • Excellent communication skills to engage with diverse individuals and partner agencies.
