We are seeking a skilled and compassionate Mental Health Practitioner to join our Homeless Health Care Team in Southampton. As a key member of our team, you will work closely with individuals who are experiencing or at risk of homelessness, providing holistic care and advocating for marginalized societal groups.
Main Responsibilities- Assess and formulate individual intervention plans for adults with complex biopsychosocial problems.
- Provide care coordination for a small caseload of service users, working in a strengths-based and recovery-focused way to support them in meeting their mental health needs.
- Develop and maintain effective relationships with a variety of individuals from diverse cultural backgrounds and partner agencies.
- Contribute to weekly multidisciplinary team meetings and work in partnership with colleagues to evaluate care plans.
- Provide supervision arrangements to ensure practitioners have opportunities to develop their skills and knowledge.
