About the Role

We are seeking a qualified Senior Mental Health Practitioner to join our team in Southampton Central Community Mental Health Team, working with adults with severe or enduring mental health needs.

The successful candidate will provide a care coordination role as a member of a multi-disciplinary team, engaging, supporting and working with a caseload of service users with severe or enduring mental health problems.

Key responsibilities include assessing and monitoring service users' mental state, providing comprehensive risk assessment and implementing a management plan, undertaking crisis management and contingency planning.

The ideal candidate will have in-depth experience of working with adults with severe or enduring mental health problems and be able to work in collaboration with service users as experts in their own care, promoting the principles of recovery, choice and personalisation.

Key Skills and Qualifications

  • Qualified as a Registered Mental Health Nurse, Social Worker or Occupational Therapist
  • In-depth experience of working with adults with severe or enduring mental health problems
  • Ability to work in collaboration with service users as experts in their own care
