About the Role

We are seeking a highly skilled Mental Health Practitioner to join our Homeless Health Care Team. As a key member of our team, you will be responsible for providing high-quality holistic care to individuals experiencing homelessness.

Main Responsibilities

  • Assess and formulate individual intervention plans for adults with complex biopsychosocial problems.
  • Work in a strengths-based and recovery-focused way to support service users to meet their mental health needs.
  • Contribute to weekly multidisciplinary team meetings and work in partnership with colleagues to evaluate care plans.
  • Provide supervision arrangements to ensure practitioners are offered opportunities to develop their skills and knowledge.

Person Specification

Essential qualifications include being a registered mental health nurse, occupational therapist, or social worker. A Disclosure and Barring Service Check will be required for this post.
