About the Role

Job Title: Senior Community Mental Health Practitioner

We are seeking a highly skilled and compassionate Senior Community Mental Health Practitioner to join our team in Southampton Central Community Mental Health Team. As a key member of our multi-disciplinary team, you will work with adults facing severe or enduring mental health challenges, providing care coordination and support to help them achieve their goals and improve their overall well-being.

Main Responsibilities:

  • Provide evidence-based interventions and advice on various areas, including housing, benefits, and social activities.
  • Conduct mental state assessments, risk assessments, crisis management, and provide therapeutic interventions.
  • Collaborate with service users to promote principles of recovery, choice, and empowerment.

Requirements:
