About the Role

We are seeking a qualified Senior Community Mental Health Practitioner to join our team in Southampton Central Community Mental Health Team, working with adults with severe or enduring mental health needs.

As a member of our multi-disciplinary team, you will provide a care coordination role, engaging, supporting and working with a caseload of service users with severe or enduring mental health problems.

You will help service users manage their symptoms, deliver evidence-based interventions and give advice and support on issues such as housing, benefits, education, employment, medication and social activities in the local area.

You will assess and monitor service users' mental state, provide comprehensive risk assessment and implement a management plan, undertake crisis management and contingency planning.

You will provide therapeutic interventions in line with the agreed care plan and risk assessment, utilising psycho-social interventions and work on improving physical health.
