Band 6 Community Sister: Hambleton and Richmondshire District Nursing
We are seeking a Registered Nurse with a Specialist Practitioner in District Nursing qualification to fill a Community Sister position aligned to Hambleton North Primary Care Network (PCN) based at Mowbray House Surgery in Northallerton.
The successful candidate will lead and manage a community nursing team, working in a multi-professional environment and contributing to the delivery of high-quality patient care.
Main ResponsibilitiesKey Responsibilities:
- Lead and manage a community nursing team to deliver high-quality patient care
- Work collaboratively with GP practices and other partners to support patients with complex needs
- Develop and implement effective care plans to prevent unnecessary hospital admissions
- Contribute to the development of the Primary Care Network and its services
South Tees Hospitals NHS Foundation Trust offers leadership and improvement training to all new staff, including a four-and-a-half-day leadership development programme and a Quality Improvement Programme.
The successful candidate will be expected to attend these programmes and develop their leadership and quality improvement skills.
Person SpecificationEssential Qualifications:
- Degree or Diploma awarded by a professional body
- Registered as fit to practice by the Nursing and Midwifery Council (NMC)
- Evidence of postgraduate clinical training or development
- Specialist Practitioner qualification in District Nursing (BSc or PGDip)
Essential Knowledge and Skills:
- Ability to work independently and as part of a team
- Ability to travel independently throughout the community
