About the Role

We are seeking a dedicated and compassionate Support Worker to join our team at Home Group, supporting an individual with complex needs in Swindon, Wiltshire.

Key Responsibilities
  • Provide person-centred support to our customer, promoting independence and social inclusion through a strengths-based approach.
  • Assist with daily living activities, including personal care, meal preparation, and household management.
  • Support our customer in developing and maintaining relationships with family and friends.
  • Monitor and report any changes in our customer's mental and physical health, ensuring their safety and well-being.
  • Collaborate with our clinical team and colleagues to develop and implement individualised support plans.
Requirements
  • Experience working in a complex care setting, with a Level 2 Diploma in Adult Health & Social Care or willingness to work towards it.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with our customer and colleagues.
  • A patient and calm approach, with the ability to adapt to changing situations and needs.
  • Knowledge of Positive Behavioural Support principles and practices.
  • Ability to work flexibly, including evenings, weekends, and bank holidays.
