Job Title: Respiratory Nurse Specialist

We are seeking a compassionate and motivated Respiratory Nurse Specialist to join our team at Leeds Community Healthcare NHS Trust. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality care to patients with Chronic Obstructive Pulmonary Disease (COPD) in their homes and clinics.

Main Responsibilities:
  • Deliver patient-centered care to individuals with COPD, empowering them to manage their condition and make informed decisions about their health.
  • Work collaboratively with colleagues in local hospitals to facilitate hospital discharges and support admission avoidance.
  • Contribute to service development and improvement initiatives, ensuring that our patients receive the best possible care.
  • Travel independently across the city, using your own vehicle for business purposes, to provide care to patients in their homes.

Requirements:
  • Experience of caring for patients with COPD, with a strong understanding of the condition and its management.
  • Ability to work independently and as part of a multidisciplinary team, with excellent communication and interpersonal skills.
  • Flexibility to work a variety of shifts, including weekends, to meet the needs of our patients.
  • Access to a suitable vehicle for business purposes, with a full driving license.
