Job Title: Clinical Case Manager

We are seeking a highly skilled Clinical Case Manager to join our team at Compass Associates. As a Clinical Case Manager, you will be responsible for managing a caseload of children and young people with complex rehabilitation needs due to backgrounds of brain, spinal or catastrophic injuries.

Key Responsibilities:
  • Assess risk and set goals with clients to ensure they receive the best possible care and rehabilitation
  • Carry out assessments, monitor progress, and meet with solicitors, MDT members, families, and external professionals
  • Develop and implement comprehensive care plans to meet the needs of clients
  • Provide high-quality reports for use in court cases
Requirements:
  • Recognised Nursing, Social Work, or Therapy degree (e.g. Speech & Language Therapy, Occupational Therapy, or Physiotherapy)
  • Full registration with either the HCPC, NMC, or SWE
  • Strong clinical knowledge and experience working within children's complex care
  • UK driving licence and access to own car
