Advanced Nurse Practitioner/Advanced Clinical Practitioner

The Park Avenue Medical Centre is seeking an experienced Advanced Nurse Practitioner/Advanced Clinical Practitioner to join our team. As a key member of our primary care team, you will provide high-quality patient care, working collaboratively with our healthcare professionals to deliver exceptional patient outcomes.
Key ResponsibilitiesClinical Practice- Conduct comprehensive clinical assessments and develop effective treatment plans for patients with acute and chronic conditions.
- Prescribe and review medication, ensuring evidence-based practice and adherence to national protocols.
- Work with patients to promote healthy lifestyles and self-care, providing education and support as needed.
- Communicate effectively with patients, carers, and colleagues, using sensitive communication styles to ensure informed consent and optimal patient outcomes.
- Utilize effective communication strategies to ensure patients are fully informed and engaged in their care.
- Communicate with patients who require bad news, using empathy and compassion to support their emotional well-being.
- Maintain effective communication within the practice environment and with external stakeholders, ensuring seamless care coordination.
- Work within your scope of practice, adhering to the NMC code of conduct and professional standards.
- Comply with NMC revalidation processes and participate in quality governance systems to ensure high-quality patient care.
- Produce accurate, contemporaneous records of patient consultations, adhering to legislation, policies, and procedures.
- Act as a clinical leader, providing daily support to the duty team and Practice Management in the delivery of same-day services.
- Problem-solve clinical matters, supporting the nursing team to deliver a quality service and communicating with Directors as needed.
- Take the lead in documenting, investigating, and reporting significant events, ensuring patient safety and well-being.
- Work effectively as a team member, supporting others and exploring new ways of working to improve patient care.
- Delegate tasks clearly and appropriately, adopting safe practice principles and assessing competence.
- Manage and assess risk within your areas of responsibility, ensuring adequate measures are in place to protect staff and patients.
- Monitor work areas and practices to ensure they are safe and free from hazards, adhering to health, safety, and security legislation, policies, and procedures.
- Undertake mentorship for junior staff, assessing competence against set standards.
- Disseminate learning and information gained to other team members, promoting good practice and informing others about current and future developments.
- Identify patterns of discrimination and take action to overcome them, promoting diversity and equality of opportunity.
- Enable others to promote equality and diversity in a non-discriminatory culture, supporting people who need assistance in exercising their rights.
