We are seeking an experienced Advanced Nurse Practitioner to join our Learning Disability Service, working across Northamptonshire to support primary care services and undertake Annual Health Checks for 14+ with a Learning Disability.
Main Responsibilities- Combine high-level assessment to make a diagnosis, initiate medical and/or nursing treatments, refer appropriately, and lead the coordination of care working with partner agencies and key stakeholders.
- Support colleagues and be responsible for non-medical prescribing, managing an age-inclusive caseload, facilitating clinics, and using a holistic clinical assessment approach.
- Manage the implementation of a range of care interventions for service users presenting with undifferentiated or undiagnosed problems.
Northamptonshire Healthcare NHS Foundation Trust is an integrated primary care and mental health Trust, providing physical, mental health, and specialty services in both hospital settings and out in the community.
Person Specification- Evidence of significant experience working in a healthcare setting at a senior clinical level.
- Independent Nurse Prescriber.
- Knowledgeable in relation to clinical governance and prescribing.
- RGN.
- Data analysis, identifies, and uses appropriate analytical methods to ensure the successful analysis of data.
- Knowledge of change management tools and able to demonstrate ability to influence change and affect change through others.
- Understanding of practices that promote self-care management, behavior change, and independent living.
- Understanding/Experience of patients with Learning Disabilities.
