Advanced Nurse Practitioner/Advanced Clinical Practitioner Role

Job Summary
The Park Avenue Medical Centre is seeking an experienced Advanced Nurse Practitioner/Advanced Clinical Practitioner to join our team. As a key member of our primary care team, you will be responsible for providing high-quality patient care, working collaboratively with our healthcare professionals to deliver evidence-based care.
Key Responsibilities
Clinical Practice
- Conduct holistic clinical assessments and develop management plans for patients with acute and chronic physical and mental health conditions.
- Proactively identify, diagnose, and manage treatment plans for patients at risk of developing long-term conditions.
- Prescribe and review medication for therapeutic effectiveness, ensuring adherence to evidence-based practice and national protocols.
- Work with patients to support compliance with and adherence to prescribed treatments, providing information and advice on medication regimens, side-effects, and interactions.
- Prioritize health problems and intervene appropriately to assist patients in complex, urgent, or emergency situations.
- Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care.
Communication
- Utilize sensitive communication styles to ensure patients are fully informed and consent to treatment.
- Communicate effectively with patients and carers, recognizing the need for alternative methods of communication to overcome different levels of understanding, cultural background, and preferred ways of communicating.
- Anticipate barriers to communication and take action to improve communication.
- Maintain effective communication within the practice environment and with external stakeholders.
Delivering a Quality Service
- Recognize and work within own competence and professional code of conduct as regulated by the NMC to deliver evidence-based care.
- Comply with NMC revalidation process, company appraisal process, and participate in the maintenance of quality governance systems and processes across the organization and its activities.
- Produce accurate, contemporaneous, complete records of patient consultation, consistent with legislation, policies, and procedures.
- Prioritize, organize, and manage own workload in a manner that maintains and promotes quality.
Leadership, Personal and People Development
- Act as a clinical leader to provide daily support alongside the duty team and Practice Management in the delivery of same-day services.
- Problem-solve clinical matters arising and support the nursing team to deliver a quality service, communicating and providing feedback to Directors.
- Take the lead to document, investigate, and report on significant events.
- Take responsibility for own learning and performance, including participating in clinical supervision and acting as a positive role model.
Team Working
- Work as an effective and responsible team member, supporting others and exploring mechanisms to develop new ways of working.
- Delegate clearly and appropriately, adopting the principles of safe practice and assessment of competence.
- Prioritize own workload and ensure effective time-management strategies are embedded within the culture of the team.
Management of Risk
- Manage and assess risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients.
- Monitor work areas and practices to ensure they are safe and free from hazards and conform to health, safety, and security legislation, policies, procedures, and guidelines.
Learning and Development
- Undertake mentorship for more junior staff, assessing competence against set standards.
- Disseminate learning and information gained to other team members to share good practice and inform others about current and future developments.

- Identify patterns of discrimination and take action to overcome this and promote diversity and equality of opportunity.
- Enable others to promote equality and diversity in a non-discriminatory culture.
