Job Summary
The Park Avenue Medical Centre is seeking an experienced Advanced Nurse Practitioner/Advanced Clinical Practitioner to join our team. As a key member of our primary care team, you will provide high-quality patient care, working collaboratively with our healthcare professionals to deliver exceptional patient outcomes.
Key Responsibilities
Clinical Practice
- Conduct comprehensive clinical assessments and develop effective management plans for patients with acute and chronic conditions.
- Prescribe and review medication, ensuring evidence-based practice and adherence to national protocols.
- Work with patients to promote healthy lifestyles and self-care, providing education and support to optimize treatment outcomes.
- Prioritize health problems and intervene appropriately in emergency situations, ensuring effective emergency care.
Communication
- Utilize sensitive communication styles to ensure patients are fully informed and consent to treatment.
- Communicate effectively with patients and carers, recognizing the need for alternative methods of communication.
- Anticipate and address barriers to communication, ensuring seamless care delivery.
Delivering a Quality Service
- Recognize and work within your scope of practice, adhering to the NMC code of conduct.
- Comply with NMC revalidation and appraisal processes, contributing to quality governance systems.
- Produce accurate, contemporaneous records of patient consultations, consistent with legislation and policies.

- Act as a clinical leader, providing daily support to the duty team and Practice Management.
- Problem-solve clinical matters, supporting the nursing team to deliver a quality service.
- Take the lead in documenting, investigating, and reporting significant events.

- Delegate clearly and appropriately, adopting safe practice principles.

Person Specification
Essential
- Registration with the NMC/HCPC.
- Evidence of education at level 7 or higher.
- Non-Medical Prescriber qualification.
- Experience working clinically as an advanced practitioner in primary or emergency care.
