Job Summary

The Park Avenue Medical Centre is seeking an experienced Advanced Nurse Practitioner/Advanced Clinical Practitioner to join our team. As a key member of our primary care team, you will provide high-quality patient care, working collaboratively with our healthcare professionals to deliver exceptional patient outcomes.

Key Responsibilities

Clinical Practice

  • Conduct comprehensive clinical assessments and develop effective treatment plans for patients with acute and chronic conditions.
  • Prescribe and review medication, ensuring evidence-based practice and adherence to national protocols.
  • Work with patients to promote healthy lifestyles and self-care, providing education and support to optimize health outcomes.
  • Prioritize health problems and intervene in emergency situations, providing effective emergency care.
  • Communicate effectively with patients, carers, and colleagues, using sensitive communication styles to ensure informed consent and optimal care.

Communication

  • Utilize effective communication strategies to ensure patients are fully informed and engaged in their care.
  • Communicate with patients who are receiving bad news, providing empathy and support.
  • Anticipate and address barriers to communication, ensuring optimal care delivery.
  • Maintain effective communication within the practice environment and with external stakeholders.

Delivering a Quality Service

  • Work within your scope of practice, adhering to the NMC code of conduct and professional standards.
  • Comply with NMC revalidation and appraisal processes, participating in quality governance systems and processes.
  • Produce accurate, contemporaneous records of patient consultations, adhering to legislation and practice policies.
  • Prioritize and manage your workload to maintain and promote quality care delivery.

Leadership, Personal and People Development

  • Act as a clinical leader, providing daily support to the duty team and Practice Management in delivering same-day services.
  • Problem-solve clinical matters, supporting the nursing team to deliver a quality service and communicating with Directors.
  • Take the lead in documenting, investigating, and reporting significant events.
  • Take responsibility for your own learning and performance, participating in clinical supervision and acting as a positive role model.

Team Working

  • Work as an effective and responsible team member, supporting others and exploring new ways of working.
  • Delegate clearly and appropriately, adopting safe practice and assessing competence.
  • Prioritize your workload and ensure effective time-management strategies are embedded within the team culture.

Management of Risk

  • Manage and assess risk within your areas of responsibility, ensuring adequate measures are in place to protect staff and patients.
  • Monitor work areas and practices to ensure they are safe and free from hazards, conforming to health, safety, and security legislation, policies, and guidelines.

Learning and Development

  • Undertake mentorship for more junior staff, assessing competence against set standards.
  • Disseminate learning and information gained to other team members, sharing good practice and informing others about current and future developments.
  • Assess your own learning needs and undertake learning as appropriate.
