Advanced Nurse Practitioner/Advanced Clinical Practitioner

The Park Avenue Medical Centre is seeking a highly skilled and experienced Advanced Nurse Practitioner/Advanced Clinical Practitioner to join our team. As a key member of our clinical leadership team, you will be responsible for providing high-quality patient care and leading the delivery of clinical services.
Key Responsibilities- Clinical Practice
- Conduct comprehensive clinical assessments and develop treatment plans for patients with acute and chronic conditions.
- Prescribe and review medication, ensuring adherence to evidence-based practice and national protocols.
- Work collaboratively with patients to promote healthy lifestyles and self-care strategies.
- Provide sensitive communication and support to patients, including those receiving bad news.
- Communication
- Utilize effective communication styles to ensure patients are fully informed and consent to treatment.
- Communicate with patients and carers, recognizing the need for alternative methods of communication.
- Anticipate and address barriers to communication.
- Delivering a Quality Service
- Work within your scope of practice and professional code of conduct to deliver evidence-based care.
- Comply with revalidation processes and participate in quality governance systems.
- Produce accurate and contemporaneous patient records.
- Prioritize and manage your workload to maintain quality standards.

- Act as a clinical leader, providing daily support to the duty team and Practice Management.
- Problem-solve clinical matters and support the nursing team to deliver a quality service.
- Take the lead in documenting, investigating, and reporting significant events.
