Job Summary

The Park Avenue Medical Centre is seeking a highly skilled and experienced Advanced Nurse Practitioner/Advanced Clinical Practitioner to join our team. As a key member of our clinical leadership team, you will be responsible for providing high-quality patient care and leading the delivery of clinical services.

Key Responsibilities
  • Clinical Practice
    • Conduct comprehensive clinical assessments and develop treatment plans for patients with acute and chronic conditions.
    • Prescribe and review medication, ensuring adherence to evidence-based practice and national protocols.
    • Work collaboratively with patients to promote healthy lifestyles and self-care strategies.
    • Provide sensitive communication and support to patients, including those receiving bad news.
  • Communication
    • Utilize effective communication styles to ensure patients are fully informed and consent to treatment.
    • Communicate with patients and carers, recognizing the need for alternative methods of communication.
    • Anticipate and address barriers to communication.
  • Delivering a Quality Service
    • Work within your scope of practice and professional code of conduct to deliver evidence-based care.
    • Comply with revalidation processes and participate in quality governance systems.
    • Produce accurate and contemporaneous patient records.
    • Prioritize and manage your workload to maintain quality standards.
  • Leadership, Personal and People Development
    • Act as a clinical leader, providing daily support to the duty team and Practice Management.
    • Problem-solve clinical matters and support the nursing team to deliver a quality service.
    • Take the lead in documenting, investigating, and reporting significant events.
    • Support staff development and promote a learning environment.

    • Work effectively with others to define values, direction, and policies impacting care delivery.
