Job Summary

We are seeking a highly skilled and experienced Practice Nurse to join our team at Primary Care Management Solutions Ltd. The successful candidate will be responsible for delivering high-quality nursing services to our patients, working closely with our multidisciplinary team to provide comprehensive care.

Main Responsibilities
  • Develop and implement health promotion and well-being programs to improve patient outcomes.
  • Implement and evaluate individual treatment plans for patients with chronic diseases, ensuring effective management and prevention of complications.
  • Identify and support patients at risk of developing long-term conditions, providing timely interventions to prevent adverse effects on their health.
  • Provide routine nursing care to patients, adhering to clinical guidelines and best practices.
  • Manage and support patients with wounds, providing specialized care and treatment.
  • Provide travel medicine services, including vaccinations and health advice.
  • Request and process pathology tests, ensuring accurate results and effective communication with patients.
  • Support the management team in reviewing clinical policies and procedures, ensuring compliance with national guidelines and best practices.
Secondary Responsibilities
  • Act as the audit lead, utilizing the audit cycle to drive quality improvement and patient safety.
  • Monitor and ensure the safe storage, rotation, and disposal of medications.
  • Support junior members of the nursing team, providing guidance and mentorship as needed.
  • Participate in local initiatives to enhance service delivery and patient care, contributing to the development of innovative solutions.
  • Support and participate in shared learning within the practice, promoting a culture of continuous improvement and professional development.

Qualifications:

  • Registered Nurse

Experience:

  • Experience working in a primary care environment
  • Experience in chronic disease management

Clinical Knowledge:

  • Wound care and removal of sutures and staples
  • ECGs and venepuncture
  • New patient medicals and chaperone procedure
  • Requesting pathology tests and processing results
  • Travel medicine and vaccinations
  • Diabetes, hypertension, asthma, and spirometry
  • CHD and immunizations
  • Women's health and cervical cytology
  • Understanding of evidence-based practice and clinical governance
  • Ability to record accurate clinical notes and work within own scope of practice
