About the Role

We are seeking a highly skilled and motivated Community Staff Nurse to join our team at Buckinghamshire Healthcare NHS Trust. As a Community Staff Nurse, you will play a vital role in delivering high-quality patient care and contributing to the development of our community nursing services.

Key Responsibilities
  • Contribute to the development and implementation of community nursing services, working closely with healthcare assistants and clinicians to provide nursing interventions to patients in their home environment.
  • Assess the physical and psycho-social needs of patients and deliver therapeutic interventions based on best available evidence to improve health outcomes.
  • Manage your own caseload, delivering and evaluating service delivery to patients with a variety of medical conditions and those delegated by case managers.
  • Participate in the referral management process to ensure patients are treated by the most appropriate member of the team/service.
  • Contribute to the development of protocols and procedures for clinical pathways.
Requirements
  • Evidence of recent CPD, including postgraduate modules in long-term conditions.
  • Demonstrate experience post or pre-registration placements in relevant clinical areas, including palliative care and catheterisation.
  • At least 1 year of community experience.
  • Knowledge of working with the common assessment process and electronic clinical systems.
