Job Summary

We are seeking a highly skilled and experienced Practice Nurse to join our team at Primary Care Management Solutions Ltd. As a Practice Nurse, you will play a vital role in delivering high-quality patient care and contributing to the success of our primary care services.

Main Responsibilities

  • Develop and implement health promotion and well-being programs to improve patient outcomes
  • Provide routine nursing care to patients, including wound care, travel medicine, and chronic disease management
  • Work collaboratively with the multidisciplinary team to provide comprehensive patient care
  • Conduct health assessments, develop treatment plans, and provide education to patients
  • Participate in quality improvement initiatives and contribute to the development of clinical policies and procedures
  • Support the management team in reviewing clinical policy and procedure

Secondary Responsibilities

  • Act as the audit lead, effectively utilizing the audit cycle
  • Monitor and ensure the safe storage, rotation, and disposal of medicaments
  • Support junior members of the nursing team, providing guidance and mentorship
  • Participate in local initiatives to enhance service delivery and patient care
  • Support and participate in shared learning within the practice

Person Specification

  • Registered Nurse with a valid registration
  • Postgraduate diploma or degree in Primary Care (desirable)
  • Experience working in a primary care environment, with a focus on chronic disease management
  • Strong clinical knowledge and skills in areas such as wound care, travel medicine, and chronic disease management
  • Ability to work autonomously and as part of a team
  • Excellent communication and interpersonal skills
