We are seeking a skilled and dedicated Health Visitor to join our team at Stockport NHS Foundation Trust. As a Health Visitor, you will play a vital role in improving the health and wellbeing of children and families in Stockport.
Key Responsibilities- Lead the Healthy Child Programme 0-5 and manage a named health visitor mixed caseload of universal, enhanced and specialised pathway families.
- Provide holistic assessment using the My World Triangle and utilise planning skills to identify and address health needs in a defined local population.
- Provide early help support to families when required, utilising the Early Help Assessment and within a Team Around the Early Years model.
- Maintain a focus on Public Health priorities, using a community strengths-based approach to reduce inequalities, utilising family hub networks across the neighbourhood you are working within.
