About the RoleWe are looking for a skilled and compassionate Rehabilitation Case Manager to work with our clients and their families. Your primary responsibility will be to assess and understand the specific needs of each child or young person and develop a personalized rehabilitation plan to maximize their recovery.
Key Responsibilities- Conduct thorough assessments of children and young people with life-changing injuries
- Develop and implement individualized rehabilitation plans
- Collaborate with other stakeholders, including solicitors and insurers, to secure funding for private and augmentative interventions
- Provide ongoing support and guidance to clients and their families
- Monitor progress and adjust rehabilitation plans as needed
- Qualification in Nursing, Physiotherapy, or Occupational Therapy
- Minimum of five years of clinical experience in paediatric rehabilitation
- Experience in rehabilitation case management
- Experience in the personal injury rehabilitation market
