Job Summary

The Southend East Primary Care Network is seeking a part-time pharmacist to join our team and contribute to the delivery of high-quality patient care. As a key member of our pharmacy team, you will play a vital role in achieving our Medicines Optimisation Local Enhanced Service (MOLES) targets.

Main Responsibilities
  • Manage medicines after discharge from hospital to ensure safe and effective care for patients.
  • Conduct risk stratification to identify patients at high risk of harm from medicines and develop strategies to mitigate these risks.
  • Unplanned hospital admissions: identify patients at risk of unplanned hospital admissions due to medicines and work with the team to develop interventions to prevent these admissions.
  • Repeat prescribing: manage the repeat prescribing reauthorisation process by reviewing patient requests and ensuring that patients receive the most appropriate medication.
  • Patient-facing medicines support: provide expert advice and support to patients on their medication, including addressing any concerns or questions they may have.
  • Care home medication reviews: conduct regular medication reviews for patients in care homes to ensure that their medication is safe and effective.
  • Long-term condition clinics: work with patients with long-term conditions to develop and implement personalized medication plans that meet their individual needs.
  • Service development: contribute to the development and management of new services built around new medicines or NICE guidance.

Person Specification
  • Mandatory registration with the General Pharmaceutical Council.
  • Excellent interpersonal, influencing, and negotiating skills.
  • Excellent communication skills, both written and verbal.
  • Ability to communicate complex and sensitive information in an understandable form to a variety of audiences.
  • Ability to plan, manage, monitor, advise, and review general medicine optimisation issues in core areas for long-term conditions.
  • Ability to obtain and analyse complex technical information.
  • Ability to work effectively independently and as a team member.
