About the Role

We are seeking an experienced and skilled professional to join our team as a Primary Care Lead at HMP Aylesbury. As a key member of our healthcare team, you will be responsible for providing clinical leadership and management of primary care services within the prison.

Key Responsibilities
  • Provide clinical leadership and management of primary care services, ensuring high-quality patient care and effective service delivery.
  • Develop and implement policies and procedures to ensure compliance with regulatory requirements and best practice.
  • Collaborate with multidisciplinary teams to ensure seamless service delivery and effective communication.
  • Manage and develop the primary care team, providing guidance and support to ensure staff are equipped to deliver high-quality care.
  • Ensure effective use of resources, managing budgets and prioritizing services to meet the needs of patients.
  • Develop and maintain relationships with external partners, including healthcare providers and community services.
  • Contribute to the development of healthcare strategies and policies, ensuring alignment with Trust objectives.
  • Provide leadership and guidance to staff, promoting a culture of excellence and continuous improvement.
  • Ensure compliance with health and safety regulations, infection control policies, and other relevant standards.
  • Manage and investigate incidents, ensuring timely and effective resolution.
Requirements
  • Registered General Nurse with significant experience in primary care.
  • Management qualifications and experience of supervising staff.
  • Knowledge of chronic disease and long-term condition management, as well as current developments and practice.
  • Ability to communicate effectively with a wide range of people, including patients, staff, and external partners.
  • Strong leadership and management skills, with the ability to motivate and develop staff.
  • Ability to work flexibly, including evenings and weekends, as required.
