Job Overview

Berkshire Healthcare NHS Foundation Trust is seeking an experienced and enthusiastic nurse to join our team as a District Community Nursing Sister/Charge Nurse. This is an exciting opportunity to work in a dynamic and supportive environment, providing high-quality care to patients in their own homes.

Main Responsibilities
  • Provide support and leadership to a great team, working together to deliver excellent patient care.
  • Manage a complex and diverse caseload, actively working to prevent admission to the acute sector and supporting patients in their preferred place of care.
  • Maintain detailed, accurate, and confidential patient records, adhering to Trust policies.
  • Lead an effective supportive team, working together to provide clinical advice and support to other staff and patients.
  • Responsible for the management of a defined caseload of patients, with continuing responsibility for the assessment of care needs, the development, implementation, and evaluation of care programmers.
Requirements
  • Registered Nurse – Adult
  • Active on the Mentor Register
  • Evidence of Leadership & Management development
  • Recognised coaching qualification or willing to undertake
