Job Overview

Berkshire Healthcare NHS Foundation Trust is seeking an experienced and enthusiastic nurse to join our team as a District Community Nursing Sister/Charge Nurse. Main Responsibilities
  • Provide leadership and support to a team of community nurses, ensuring the delivery of high-quality patient care.
  • Manage a complex and diverse caseload, actively working to prevent admission to the acute sector and supporting patients in their preferred place of care.
  • Develop and implement care programs, ensuring that patients receive evidence-based care that meets their individual needs.
  • Collaborate with other healthcare professionals to ensure seamless care delivery and promote a holistic approach to patient care.
  • Conduct regular audits and evaluations to ensure that care standards are met and identify areas for improvement.
Requirements
  • Registered Nurse - Adult qualification and NMC registration.
  • Active on the Mentor Register.
  • Evidence of leadership and management development.
  • Recognised coaching qualification or willingness to undertake.
  • At least 2 years' experience in community nursing at Band 6.
