Main Responsibilities- Provide leadership and support to a team of community nurses, ensuring the delivery of high-quality patient care.
- Manage a complex and diverse caseload, working to prevent admission to the acute sector and supporting patients in their preferred place of care.
- Develop and implement care programs, ensuring the needs of patients are met and their care is coordinated.
- Collaborate with other healthcare professionals to ensure seamless delivery of care.
- Conduct risk assessments and develop plans to mitigate risks, ensuring patient safety.
- Registered Nurse - Adult qualification and NMC registration.
- Active on the Mentor Register.
- Evidence of leadership and management development.
- Recognised coaching qualification or willingness to undertake.
- At least 2 years' experience in community nursing, preferably at Band 6.
