About the Role:

We are seeking a highly motivated and experienced Community Staff Nurse to join our team at Berkshire Healthcare NHS Foundation Trust. Key Responsibilities:

  • Visiting patients in their own homes to provide nursing care and support
  • Assessing patients' needs and developing individual care plans
  • Working collaboratively with other healthcare professionals to ensure seamless care delivery
  • Providing education and support to patients and their families
  • Participating in quality improvement initiatives to enhance patient care

Requirements:

  • Registered General Nurse (RGN) qualification and NMC registration
  • Experience of working in community nursing or a similar setting
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
  • Strong organizational and time management skills
